Before submitting a request for your sacramental record:
Please first visit or contact the parish office where you received your sacrament.
You can find your parish information on our home page, under Parish Finder.
Click on Parish Finder, and search for your parish/village name and click on details to get their contact number and email address.
Access
Requests for a certificate must be submitted in writing to the Chancery or via email. For privacy protection of the individual involved, certificates are only issued to the person whose record it is, or to a person who provides proof of parental authority, guardianship, power of attorney, executor/personal representative or other legal status on behalf of the person whose record is requested, and clergy, parish, or Archdiocesan personnel in need of records for official purposes.
The Archdiocesan archives are NOT open at this time to those engaged in genealogical or family history studies. Please see other resources.

Indicate in your request the following information, a copy of your photo identification, and proof of your relationship to the individual (if applicable).
Sacramental record(s) requested: Baptism, First Communion, Confirmation, Marriage, other. If requesting marriage certificate, provide name of spouse. *
It may take up to 14 working days to complete a search and produce documentation.
